Student Emergency Information
Dear Parents,

Student emergency contact and medical information will not be done online this school year.

To ensure your child’s safety while at school, parents must complete a Student Emergency Information form for each child attending school.  The form is on the New Haven High School web page under the Parents tab. You may print the form off and complete at home prior to coming to registration; or you may complete a Student Emergency Information form for each child attending school at registration.  Having completed forms prior to attending the New Haven High School registration will ensure a quicker registration process.

Our school nurse will be at registration to collect the Student Emergency Information forms and be available for questions or to discuss your child’s medical concerns.
